Every agreed action needs one named person, a realistic deadline and a review point. That person does not have to complete every part of the work; they are responsible for making sure the next step happens and returns to review.
Clear ownership is supportive rather than punitive. It protects staff from duplicated effort, reduces missed follow-up and gives families a more coherent experience.
At the end of each attendance meeting, test the record: Is every action specific? Is one person named? Is the review date clear? Do we know what evidence of impact we expect to see?
